Steps
Step 1: Preserve the transcript and meeting context
Export or copy the complete transcript, meeting date, participant list and any available timestamps. Keep the original unchanged so later edits can be checked against it. Remove information your organization has not approved for the processing environment. Before extraction, note transcription gaps, uncertain speaker labels and sections where audio quality makes the wording unreliable. The micro-outcome is a stable source record, not a polished summary. Verify this step by opening three random transcript passages and confirming their timestamps and speaker names still match the original recording or meeting record.

Run the accountable-action prompt against the preserved transcript. Require one row per commitment with task, stated owner, stated deadline, supporting passage or timestamp, and a confidence flag. If the transcript implies responsibility but does not assign it, write “owner unresolved.” If a date is relative, retain the original phrase and calculate a calendar date only after confirming the meeting date. Keep proposals, requests and decisions in separate fields. Verification succeeds when every row points back to language that actually commits someone to an action.
Step 3: Separate decisions, risks and open questions
Run the classification prompt on the same source and compare it with the action table. A decision records an agreed choice; a risk records a condition that may affect delivery; an open question records information still needed. Do not convert any of these into a task unless the transcript also contains an explicit commitment. Merge duplicate statements only when they describe the same work and preserve the strongest source reference. Verify the separation by checking that a reader can tell what was decided, what remains uncertain and what someone must do next.
Step 4: Audit owners, dates and duplicates
Use the audit prompt on the draft tracker, then perform a manual source check. Look specifically for invented owners, deadlines inferred from urgency, repeated tasks phrased differently and decisions mislabelled as actions. Resolve contradictions by returning to the transcript, not by choosing the most plausible wording. Mark unresolved fields for participant confirmation. The step is complete when the audit log records every correction and the tracker contains no silent assumptions. Keep rejected rows in the audit log so a later reviewer can see why they were excluded.
Step 5: Approve before exporting or assigning
Send the reviewed table to the meeting owner or designated reviewer. Export only approved rows to the task system, spreadsheet or automation. Configure any Zap as a draft or review step first; do not let an unreviewed model output assign work or notify participants. Include the source link or timestamp in the destination when the system supports it. Verify the export by comparing the count, owner, deadline and source reference of every destination row with the approved tracker. A successful automation run is not proof that the content is correct.
